1. Before you call to make an appointment make sure to provide the following information:
a. Full Name
b. Date of birth
c. Email address and contact telephone number
d. Reason for your visit (medical or cosmetic)
e. Valid credit card to secure your appointment
f. Name of your Primary Physician
2. Call to schedule your appointment 917-853-3376 (DERM)
3. You can print out the patient registration forms, fill in the required information and bring those with you on the date of your appointment.
Confirm your Appointment
We send an email reminder/telephone call a few days prior to your scheduled appointment date to confirm your appointment. If you cannot be reached we will leave a message for you. Please call us back to confirm the appointment.
Cancellation Policy:
Dr Khan’s office has a 24 hour cancellation policy. Should you chose to cancel or change your appointment time please call us at least 24 hours in advance to avoid the cancellation fee. The cancellation fee is $250 for medical appointments and $400 for cosmetic appointments. The cancelation fees apply to surgical procedures for which an OR was reserved for you. Those fees vary from depending upon the nature of the procedure.
Your office visit:
Bring your referral letter if applicable for all medical appointments Appointments concerning cosmetic procedures are not billed to the insurance and referral letters are not required Arrive at least 15 minutes prior to your appointment time to fill in the paperwork required for your appointment All visits are applicable to a doctor’s fee as determined by the office. All fees must be paid in full at the completion of your visit, either by check, cash, or credit card(s). We accept, Visa, Master card, Discover, American Express and Care Credit.
